Reference values for lysosomal enzymes activities using dried blood spots samples - a Brazilian experience.

Lysosomal storage diseases (LSD) are inherited disorders caused by deficiency of lysosomal enzymes in which early diagnosis is essential to provide timely treatment. Exploratory study of plasma total homocysteine and its relationship to short-term outcome in acute ischaemic stroke in Nigerians.

Hyperhomocysteinemia is a potentially modifiable risk factor for stroke, and may have a negative impact on the course of ischaemic stroke. The role of hyperhomocysteinemia as it relates to stroke in Africans is still uncertain. The objective of this study was to determine the prevalence and short-term impact of hyperhomocysteinemia in Nigerians with acute ischaemic stroke. We hypothesized that Hcy levels are significantly higher than in normal controls, worsen stroke severity, and increase short-term case fatality rates following acute ischaemic stroke.

Hypofibrinolysis, thrombophilia, osteonecrosis.

In the context of additional characterization of the pathoetiologic associations of heritable hypofibrinolysis and thrombophilia with osteonecrosis of the hip, the authors assessed 15 women and 21 men at entry to a 12-week treatment study of the amelioration of Ficat Stages I or II osteonecrosis by low molecular weight heparin (Enoxaparin). All 36 patients had osteonecrosis of the hip; four patients had unifocal osteonecrosis, 25 patients had two joints affected, five had three affected joints, and two had four affected joints. In 11 of 15 women (73%), hyperestrogenemia of pregnancy (20%) or exogenous estrogen supplementation (53%) were associated with the development of osteonecrosis. Five gene mutations affecting coagulation and nine serologic coagulation tests were studied. Compared with control subjects, patients were more likely to have heterozygosity and homozygosity for the hypofibrinolytic 4G polymorphism of the plasminogen activator inhibitor-1 gene. Moreover, the plasminogen activator inhibitor-1 gene product, plasminogen activator inhibitor activity, the major determinant of hypofibrinolysis, was 10 times more likely to be high (> 21.1 U/mL) in patients than in control subjects (31% versus 3%), with a median of 15.7 versus 6.3 U/mL. Compared with controls, patients were more likely to have the thrombophilic methylenetetrahydrofolate reductase gene mutation. In addition, the thrombophilic methylenetetrahydrofolate reductase gene product, homocysteine, was four times more likely to be high (> 13.5 umol/L) in patients than in control subjects (20% versus 5%), with a median of 9.1 versus 7 umol/L. Twenty-three percent of patients had low levels (< 65%) of the thrombophilic free protein S versus 3% of control subjects. Patients were more likely than control subjects to have hypofibrinolytic high lipoprotein (a) (> or = 35 mg/dL), 33% versus 13%. Median lipoprotein (a) was higher in patients than in control subjects, 15 versus 5 mg/dL. Heritable hypofibrinolysis and thrombophilia, often augmented in women by hyperestrogenemia, seem to be major pathoetiologies of osteonecrosis. If the association between coagulation disorders and osteonecrosis reflects cause and effect, as postulated, then anticoagulation with Enoxaparin should be a promising therapy for patients with osteonecrosis.

Erythrocyte, plasma total, ultrafiltrable and platelet magnesium in type 2 (non-insulin dependent) diabetes mellitus.

We studied plasma magnesium, plasma ultrafiltrable magnesium, erythrocyte and platelet magnesium in 28 non-insulin dependent (NIDDM) patients and 28 age and sex matched non-diabetic control subjects. The plasma magnesium concentration was significantly lower in the diabetic subjects as compared with the control subjects (0.67 +/- 0.09 mmol/l versus 0.72 +/- 0.07 mmol/l, P < 0.05) as was plasma ultrafiltrable magnesium (0.43 +/- 0.06 mmol/l versus 0.47 +/- 0.05 mmol/l, P < 0.01. Similarly, erythrocyte magnesium was lower in the diabetic subjects than the non diabetic controls, (0.17 +/- 0.03 nmol/10(6) cells versus 0.19 +/- 0.02 nmol/10(6) cells, P < 0.01. Platelet magnesium was higher in the diabetic subjects (48.3 +/- 19.1 umol/g platelet protein versus 36.6 +/- 22.2 umol/g platelet protein, P < 0.01). There was no significant difference between the % plasma ultrafiltrable magnesium with respect to total plasma levels between the diabetic and normal subjects (64.7 +/- 4.7% versus 65.8 +/- 3.2% respectively). There was no significant correlation between any of these parameters and systolic or diastolic blood pressure of HbA1C in the diabetics. Nor were there any significant differences in these magnesium parameters in patients with or without diabetic complications.

Renal alteration and development of hypertension in diabetic rats.

There is a close association between diabetes and hypertension. Many studies have demonstrated an increased incidence of hypertension in the presence of diabetic nephropathy. A comparative study of the glycosylated plasma proteins in diabetic Nigerians.

The levels of glycosylated plasma proteins and haemoglobin in some Nigerian diabetic patients in Lagos metropolis have been investigated by the thiobarbaturic acid (TBA) assay technique. The mean values of the glycosylated plasma proteins: 40.8 +/- 5.89 umol/L fructose equivalent and glycosylated haemoglobin, 105.0 +/- 20.81 umol/L fructose equivalent were approximately twice that found in the controls, 23.5 +/- 3.00 and 64.7 +/- 5.87 umol/L fructose equivalent respectively. The glycosylated haemoglobin values were considerably higher in both diabetics and control than the corresponding glycosylated plasma proteins. Glycosylated plasma proteins had a high positive correlation with both plasma glucose level and glycosylated haemoglobin. These findings suggest that determination of glycosylated plasma proteins (GPP) could be employed to assess diabetic control given the shorter half-life of the plasma proteins. The GPP values are probably good indicators of integrated short-term glycaemia.
